RPM puts a cellular device in your patient’s home and a clinical team behind it — readings watched every day, a phone call when something needs attention. Medicare reimburses it monthly. Ronvida runs all of it under your practice’s name.

The month

How a month actually runs

  1. A device ships to the patient — already connected, no smartphone or Wi-Fi needed. Our team handles consent, education, and setup (the one-time 99453).
  2. Readings accumulate. The device-supply code (99454) requires 16 distinct transmission days per 30-day cycle — we track it to the day, in the patient’s own time zone, and call patients who fall behind.
  3. Our clinical team does the rounds — daily review against thresholds set per patient and approved by your physician, calls when a reading crosses its line, every minute logged. Management time (99457) needs 20 recorded minutes and at least one real-time call; 19:59 does not round up, so we never bill it.
  4. The month closes. Claims are prepared under your practice’s credentials, each line linked to the readings and minutes behind it — ready for your biller, or ours to handle end to end.

Who qualifies

Medicare Part B patients with a chronic condition that benefits from daily data — hypertension, diabetes, heart failure, COPD are the workhorses — who consent and can take a reading at home. One chronic condition is enough. We screen your panel and show you exactly who fits.

Your part

What your practice does

Approve the patient list, and a physician reviews and signs off monthly — in our console or entirely in your EMR. Enrollment, devices, monitoring, documentation, and claim preparation are ours.
