CCM is the care that happens between visits for patients with two or more chronic conditions — medications reconciled, specialists aligned, follow-ups kept. Medicare reimburses it every month. Ronvida’s team does the work; your physicians stay in charge.

The month

How a month actually runs

  1. Enrollment starts with documented consent — including the cost-sharing conversation — captured before the first billable minute.
  2. We draft the comprehensive care plan for your physician to approve. Not paperwork theater: CCM months are not billable without an active plan, and our system blocks the claim until one exists.
  3. Each calendar month, the coordination happens — medication reconciliation, patient and caregiver calls, specialist and pharmacy alignment — every minute logged to the CCM clock. The first billable threshold is 20 recorded minutes (99490); sustained months add up to two units of 99439.
  4. The month closes. Claims prepared under your practice’s credentials, every line linked to the logged time and the active care plan behind it.

Who qualifies

Medicare Part B patients with two or more chronic conditions — the classic pairs: hypertension + diabetes, heart failure + COPD, CKD + hypertension. That’s most of a typical Medicare panel; we run the screen so you don’t have to guess.

What your practice does

Approve the patient list and each care plan, then a monthly sign-off — in our console or your EMR. Everything else, including the twenty minutes themselves, is our clinical team’s work.
